| Incident Report Form | The coach should keep a record of all accidents and incidents, and notify parents/guardians and club officers where warranted. | Injury Fund | NOTE: the Injury Fund is not an insurance. It is a fund which is supported by the compulsory fee paid by playing members, which is included in their registration fee. Unregistered players will NOT be covered by the fund. There are significant restrictions on what may be claimed under the fund. You should verify what treatments are covered prior to committing to medical expenditure. Claim submissions must comply with the requirements of the Fund, which are specified on the Injury Fund website (see link over). These requirements include, but are not limited to the following.
